The Alzheimer's Society produced this practical resource for carers looking after loved ones with dementia which we have shared for World Alzheimer's Day 2022. 

The North East & Cumbria Learning Disability Network have developed a Health and Care Passport. This is an editable PDF whereby families, carers and people can fill it in electronically. This will support people to keep it regularly up to date for it to be printed and used in a health and care setting, equally it can be printed off and handwritten on. They have changed the title from hospital passport to health and care passport as it can be used in multiple settings.
 

We will be starting our second cohort of Peer Support sessions on Tuesday 19th July 1pm-3pm. We will be running 6 sessions.
