The North East & Cumbria Learning Disability Network have developed a Health and Care Passport. This is an editable PDF whereby families, carers and people can fill it in electronically. This will support people to keep it regularly up to date for it to be printed and used in a health and care setting, equally it can be printed off and handwritten on. They have changed the title from hospital passport to health and care passport as it can be used in multiple settings.
Alongside this paper version a digital version will also be made available through the Health and Wellbeing App that Julie Tucker is leading on. Updates on this will be shared in due course.
The passport has been co-produced with experts by experience and compliments the guidance on 'what to do when you are coming in to hospital' already in circulation (below). It will also be well linked to the Learning Disability and Autism Diamond Standards training the network have developed.
