Redcar & Cleveland Borough Council and Middlesbrough Council, working in partnership with NHS Tees Valley Clinical Commissioning Group (CCG) and with support and input from the South Tees Carers Forum, will be delivering a South Tees All Age Carers Support Service to the residents across the South Tees footprint.
The new service will deliver on the partnership's promise to support Carers across the South Tees area through the commissioning of a revised model of support. This approach will aim to provide a broad offer for carers, increasing the identification of Carers in the community and broadening the interventions and support offers available.
The services on offer will work together to increase the general awareness of Carers not only in communities but across all providers both statutory and third sector, but importantly it will also empower Carers to self-identify and direct themselves to a range of services available. We Care You Care will play a vital role in delivering co-ordinated communications across the South Tees area so that local Carers have information and tools readily available to connect to further support should they need to.
The new services that will be coming into effect from April 2022 are:
- Adult Carers Information and Support Service – Delivered by Carers Together
- Young Carers Information and Support Service– Delivered by Junction Foundation
- Primary Care Outreach and Support Service– Delivered by Middlesbrough and Stockton Mind
- Mental Health Community Outreach Service– Delivered by Redcar and Cleveland Mind
- Secondary Care Hospital Liaison Service– Delivered by Carers Together
