Herbert Protocol Form

People with dementia can sometimes get lost due to memory, navigation, and time confusion. The Herbert Protocol form helps carers, family, or the person themselves provide key information to the police if they go missing. It is a national scheme used by the Cleveland Police to quickly locate missing people living with dementia. 

The form is used to help the police if the person goes missing. It may include information such as places where they used to live, or places where they socialised or worked.

Keeping a completed form means you don’t have to try to remember the information when you are under stress if someone goes missing. And it saves time, so searches can start quickly.
