Emotional Wellbeing Support Service for carers across the South Tees

Teesside Mind are working together with carers and families across the South Tees to provide additional guidance, advice and emotional support around all aspects of caring for someone and the impacts this may have on people’s everyday lives.   • Support at meetings
  • Information and advice
  • Support groups and wellbeing workshops
  • Signposting to our networks across the South Tees
  • Support with assessments

Primary Care Outreach and Support Service

The aim of the service is to work with Primary Care Networks across South Tees, to encourage professionals to identify Carers and support them in their caring role.

The dedicated Primary Care Link Worker will work alongside Primary Care services to meet the aims of the NHS long-term plan to grant carers greater recognition and support so that their own health and wellbeing needs are addressed. Primary care services include general practice, Community pharmacy, dental, and optometry services, which provide the first point of contact for carers in the healthcare system.

Teesside Mind is working with primary care services to:

  • Identify carers at the first point of contact
  • Raise awareness of the We Care You Care carers information platform
  • Provide information on commissioned services or community-based services for support
  • Signpost staff to relevant carers awareness training
  • Signpost carers to relevant services for support
  • Develop Primary Care Carers Champions

You can inform your GP surgery of your caring role using one of our handy Carers GP Identification Forms
